Peter D. Rejak 65, passed away peacefully on Monday August 6, 2018.
He is survived by his beloved wife Michele, loving mother Eleano, three loyal brothers and their families. Also left to mourn are many other caring relatives and dear friends.
The family would like to thank the doctors, nurses and staff at Ellis Hospital along with Community Hospice for their compassion during this difficult time.
